This is a gunsmith or blacksmith made ball puller from the 1800's. It measures 12" in length and is made of cast iron. It would have been used to remove stuck balls in large caliber flintlock pistols, from .54 caliber up to .69 caliber. The worm appears to have been hammer forged onto the rod. Mst of the barrels on the early flint pistols were between 8" and 12" in length, so this would have worked on pretty much anything coming through the door. This would look great with an old flint pistol, shot bag, and powder horn!
